Characteristic maladaptive schemas in patients with obsessive-compulsive disorder

Abstract
Despite the development of effective pharmacological and cognitive behavioral therapies, obsessive- compulsive disorder(OCD) that often combines with deeper underlying pathologies such as personalty problems and dysfunctional schema, is still difficult to treat. On the assumption that the schema therapy would provide additional modality to help patients with OCD, we explored maladaptive schemas in patients with OCD in this study. Thirty eight patients with OCD aged from 18 to 42 years and 79 normal control subjects aged from 22 to 36 years participated in this study. Young Schema Questionnaire-Short form 3(YSQ-SF3) was used to evaluate maladaptive schemas and the Yale-Brown Obsessive Compulsive Scale(Y-BOCS), Beck Depression Inventory(BDI), Maudsley Obsessional-Compulsive Inventory(MOCI) were also used for clinical symptoms. OCD patients showed higher schema scores in Defectiveness/Shame, Vulnerability to Harm or illness, Negativity/Pessimism than normal controls when age, education and depression were controlled. Several schema scores correlated with the severity of depression and obsession as well as symptom dimensions of OCD. These results suggest that Defectiveness/Shame, Vulnerability to Harm or illness, and Negativity/Pessimism may be pronounced maladaptive schemas and strategies dealing with these schemas needs to be incorporated in the schema focused therapy for patients with OCD.
